When a conscious, competent adult patient declines medical treatment and transportation against medical advice, and refuses to sign a refusal of aid form, this fact should be documented on the form, and signed by all witnesses who observed the refusal to sign. The incident report should reference the patient refusal of aid, refusal to sign, and again reference what information was given to the patient regarding the possible consequences of the refusal.
